Matthew Greisen Student Contract

Matt Greisen Guitar, Ukulele, Piano, Bass and Drum Studio.

Agreement:

Location: All lessons will take place at Matthew Greisen’s house, over Zoom, or at students' houses (rates may vary depending on commute time).

Materials: It is recommended that students keep a three-ring binder or folder for lesson notes. Depending on the instrument and skill level, certain books or sheet music may need to be purchased for use during lessons. I am able to purchase these books for you and bring a receipt for reimbursement.

Scheduling:

A 24-hour notice is required to cancel or reschedule a lesson. Cancellations on short notice are non-refundable and will not be rescheduled.
Please inform me at the beginning of each month if your weekly time needs to be altered. I will do my best to accommodate your schedule.
Lessons will begin at the agreed-upon time and end after the allotted time with no exceptions and with full payment. I often teach students consecutively and cannot accommodate late arrivals.
If payment is not made before or at the first lesson, the lesson will not take place.

Makeup Lessons:

Makeup lessons must be scheduled within 4 weeks of the original lesson date.
If we cannot find a suitable time, we can add time to the end or beginning of lessons if no other students are scheduled immediately before or after. Alternatively, lessons canceled with more than 24 hours' notice can be refunded at 75% of the individual lesson rate.
Lessons cancelled by me can be refunded at 95% or rescheduled given availability.

Ending Lessons:

You must notify me one month in advance if you plan to discontinue lessons.
If you pay via direct deposit through my website, it is your responsibility to cancel payments before your billing date.

Payment Details:

Payment must be done through my website
Payment through my website is billed every four weeks.
Observed holidays and breaks include Christmas week(Sunday-Sunday), Thanksgiving (Wednesday to Friday), New years Eve and New Years Day, and Easter. These times are paid for even though lessons will not occur, but they can be rescheduled if there is availability, although this is not promised.
Rates are subject to change yearly, you will be given at least 1 month notice.

Health and Safety:

Students must follow all health and safety guidelines during in-person lessons, including any current COVID-19 protocols and hygiene practices.

Behavior Expectations:

Students are expected to behave respectfully and maintain focus during lessons to ensure a productive learning environment.

Liability Waiver:

Matthew Greisen is not responsible for any accidents or injuries that occur during in-person lessons at his house or the student's house.

Confidentiality:

All personal information shared during lessons will be kept confidential.

Parental Involvement:

Parents are encouraged to be involved in their child's progress, especially for younger students.
